What is a Needs Assessment?

A Needs Assessment is the process of a District Health Board (DHB) assessor evaluating how much help a person needs in their daily life.

The DHB assessor interviews the person, and asks questions about:

The DHB assessor uses the information from the interview to recommend what kind of assistance is best. The DHB assessors may recommend home help, or they may suggest entry to a rest home or hospital.

What is Service Co-ordination?

Service co-ordination is the process of the DHB assessor deciding what care a person will receive.

The DHB assessor uses the information from the needs assessment to create a plan for the person’s ongoing support. The plan outlines what services are recommended for the person’s care.

The person being assessed may receive home care, or enter a rest home or hospital.

How can my parent contact a DHB assessor?

Your parent can contact the DHB assessors in a number of ways:

Your parent can visit his or her General Practitioner (GP). Your parent’s GP will refer them to a DHB assessor. If your parent is in hospital, a nurse will arrange an appointment with the DHB assessor.

OR

You can contact the DHB assessors and make an appointment yourself. Phone your District Health Board, and ask to speak to a Needs Assessor.

OR

You can also ask a social worker or an organisation such as Age Concern to contact a DHB assessor.

Will my parent need to contact the DHB assessors if their circumstances change?

Your parent will need to contact a DHB assessor if their financial or personal circumstances change. If their financial situation has changed, they will need another financial means assessment. If their care needs have changed, they will need a reassessment. The DHB assessor will tell your parent if they need another level of care.
