How should we prepare for the assessment?

You should talk to your family member about their living situation. Look at the table below to see what topics your parent will be asked about.

Before the assessment takes place write down:

Your parent must give the DHB assessor all the information they ask for. Tell the assessor if your parent is having problems with household tasks or experiencing depression.

What topics will the DHB assessor ask about?

The DHB assessor will ask questions about:


  • managing medication
  • vision
  • pain levels
  • breathlessness
  • levels of depression
  • personal hygiene
  • mobility


  • family support available to you
  • your carer’s needs
  • equipment

Daily Tasks
